Very similar to this:, but with slight layout differences on the side B label.
Contains attached four-page photo booklet inside matt gatefold sleeve.
No “Made in England” on label.
On the label the title appears as “Black Sabbath Vol. 4”.

A2 appears as “Tomorrow Dream” on cover, but as “Tomorrow’s Dream” on label.
